Easy Apply — Passport Application Assistant

About Easy Apply

1.  Is Easy Apply a government website? Are you affiliated with the U.S. Department of State?

No. Easy Apply is a private, independent application-preparation service operated by Easy Apply. We are not affiliated with, endorsed by, or associated with the U.S. Department of State, the National Passport Information Center, or any government agency, and we are not a law firm. We provide software that helps you prepare your passport application correctly; you then submit your completed application to the government yourself. The U.S. Department of State is the only authority that reviews, approves, and issues passports.

2. What exactly does your service do?

We do three things. First, our guided questionnaire asks about your situation in plain language and determines which application form applies to you — DS-11, DS-82, or DS-64. Second, our system reviews your answers for blank fields, inconsistencies, and the errors that most commonly cause applications to be returned. Third, we deliver a complete package to your email: your prepared application printed with its machine-readable barcode, a document checklist personalized to your answers, and instructions telling you exactly where and how to submit it.

3. What don’t you do?

We do not submit your application for you. We do not courier, expedite, or hand-carry applications to passport agencies. We do not schedule appointments on your behalf. We do not issue passports, and we cannot influence whether or how quickly the Department of State processes your application. We do not provide legal advice.

4. Can I get these forms for free?

Yes. Passport application forms are available free of charge at travel.state.gov, and you are free to fill them out yourself. Our fee is for the guided preparation service, the error review, the barcode-printed output, and the personalized checklist and instructions — not for the form itself.

5. Which form will I need — DS-11, DS-82, or DS-64?

Our questionnaire determines this from your answers, but here is the short version:

Form DS-82 (Renewal) applies if *all* of the following are true: your most recent passport was issued less than 15 years ago, you were 16 or older when it was issued, you still have it and it is undamaged, and it was issued in your current name or you can document your name change.

Form DS-11 (New Application) applies if *any* of the following are true: it’s your first U.S. passport, your previous passport was issued when you were under 16, it was issued more than 15 years ago, it is damaged, it was lost or stolen, or you are applying for a child under 16.

Form DS-64 (Lost or Stolen Report) is filed alongside a DS-11 when your passport was lost or stolen and you have not yet reported it.

6. What if I answer something wrong and get the wrong form?

The questionnaire asks about the underlying facts — when your passport was issued, how old you were, whether you still have it — rather than asking you to self-diagnose which form you need. That’s the point of the design. If your circumstances change or you realize you answered something incorrectly, contact our support team.

7.  Can I use this for my child’s passport?

Yes. Applications for children under 16 use Form DS-11 and must be submitted in person, with both parents or legal guardians present, or with a notarized Form DS-3053 if one parent cannot appear. Our questionnaire covers child applications, and your checklist will list the additional documents and the appearance requirements that apply.

8. What about a name change?

If your name has changed since your passport was issued and you can document the change with a marriage certificate, divorce decree, or court order, you may still be eligible to use Form DS-82. If you cannot document it, you will need Form DS-11. Our questions cover this.

9. Can I apply for a passport card?

Yes. The passport card is a separate document valid only for land and sea travel to Canada, Mexico, Bermuda, and the Caribbean — it cannot be used for international air travel. You can apply for a card, a book, or both. The questionnaire asks which you want.

10. My passport was damaged. Which form?

A damaged passport cannot be renewed by mail. You apply as a new applicant using Form DS-11, and you must submit the damaged passport along with a signed statement describing the damage. Normal wear does not count as damage; water damage, tears, and a detached cover do.

11. How much does your service cost?

Our service fee is $69.90. This is a one-time charge. There is no subscription, no recurring billing, and no automatic renewal of any kind.

12. Does your fee include the government passport fees?

No. Our fee covers only our preparation software and support service. All government fees are separate and are paid directly to the U.S. Department of State and, where applicable, to the acceptance facility where you submit your application. Keeping these separate means you always know which part is our service and which part is the official government cost.

13. What are the government fees?

Passport Book only: $130
Passport Card only: $30
Passport Book & Passport Card: $160

Expedited service: $60.00

Express delivery: $23.36

14. How do I pay the government fees?

Not through us. You pay them when you submit your application — by check or money order to “U.S. Department of State” for the application fee, and separately to the acceptance facility for the execution fee. Your package includes instructions on this. Acceptance facilities vary in what payment methods they take, so confirm with yours in advance.

15. What is the barcode on my application?

Applications we prepare are printed with a machine-readable 2D barcode that encodes the data from your form. When your application is received, the barcode allows your information to be captured electronically rather than re-typed from the page by hand, which removes a step where transcription errors are introduced.

16. Do I still need to sign the form?

Yes. Your printed application still requires your signature. For Form DS-11, do not sign it in advance — you must sign in front of the acceptance agent. Your instructions will tell you which applies to your form.

17. How long does the questionnaire take?

Most people finish in under half an hour. The questionnaire adapts to your answers, so you only see the questions relevant to your situation.

18. What do I receive, and when?

Your package is delivered to your email address. It contains your prepared application printed with its barcode, a document checklist built from your specific answers, and step-by-step instructions for submission.

19. What documents will I need?

That depends on your form. Generally you will need proof of U.S. citizenship (a certified birth certificate or a previous passport), a photo ID and a photocopy of it, and one passport photo meeting the Department of State’s requirements. Applications for children and name-change cases require additional documents. Your personalized checklist lists exactly what applies to you.

20. Where do I submit my application?

If your form is a DS-82, you mail it. If your form is a DS-11, you must submit it in person at an acceptance facility — typically a post office, clerk of court, or public library. Your instructions tell you which applies and how to find a facility near you.

21. Can I make changes after I receive my package?

Contact our support team and we’ll help. If the change is substantive — a different form applies to you, for instance — we’ll walk you through what’s needed.

22. Is my information secure?

Yes. Your information is transmitted and stored using AES-256 encryption, and payment card data is handled entirely by our payment processor — we never see or store your card number.
